The ?-Blade (?????, K?bur?do?) is a legendary Keyblade introduced in the Kingdom Hearts prequel, Kingdom Hearts Birth by Sleep. if you meant when xigbar saw xion as ventus well that because she may of looked like ventus but he wasn't really him just xion so they wouldn't fight the same way.

After defeating braig who ran away aqua was knocked out by vanitas's surpise attack. ventus manage to unfreeze himself to stop vanitas from killing aqua. ventus then vs vanitas, after defeating him vanitas's face was finally revealed he then got the unversed to hold down ventus so that they can merge creating the x-blade. Aqua awakens under Mickey's care to find ventus under vanitas control as you see in the 3rd picture. while mickey and aqua vs vanitas in ventus's body, ventus faced vanitas at the awakening in his mind. Aqua shatters the ?-Blade as Ventus destroys Vanitas, though Ven's heart is lost in the process. sorry if that didn't make since hope i helped.

kairi lost her heart in Kh1 when Destiny Islands was attacked by shadow heartless here: Sora obtains a mysterious weapon, the Keyblade, to defend himself. Using it to fend off the creatures, he heads for a secret cave, where he finds Kairi near a door. She turns to him, saying his name as the door behind her blows open. The ensuing blast of darkness sends Kairi rushing at Sora when she vanish just before they embrace this is when she give him her heart.
