Does anyone know where to get all the Xehanort reports so I can unlock the secret ending.

Most of them are obtained through the stories ,however, some are obtained through chests and one from the Mirage Arena using Terra.

Question: I have been reading on the fourm that to unlock the secret ending that you need to collect all the Xehanort Reports. However the guide says that you must "Complete the Reports Section." Now the journal in the game is named as reports. Have we 100% confirmed that the U.S. Version is unlocked using strictly the Xehanort Reports or do we have to fill the entire Journal.
