Recently, I think it was my inability to remember my Shotgun existed in Last of Us Multiplayer. I was in many positions to use it effectively but I forgot I even had it and persisted to use my pistol.
The best? Probably when I played FF13 and had to fight Dahaka a couple years back, bought a ton of Rune Bracelets and use my gil to go towards upgrading them to Witch'sBracelets, and then checked the shop and noticed I could have bought them all for cheaper. Still failed pretty horribly. My friend was over the next day, I had to do the dishes, so I explained the mechanics and such, and I come back in to check after like ten minutes and he beat the boss on his first firetrucking try.

Probably when I was younger, playing CoM on the Gameboy. I spent days trying to figure out how to get past Squall....and his somersault tutorial. For some reason I didn't know how to press two buttons at the same time....

1st save file on KH ever. Not equipping the ability "Scan" until getting to Hollow Bastion and wondering "why" they would make a game without the ability to see an enemy's HP. Literally facepalmed for like ten minutes.

Not sure if that really count as a fail,but still:

 

The first Megaman game I've ever played was Megaman X3 and back then I know nothing about Megaman expect that its games are HARD.For some reason when I reached the Stage select screen I though that the easiest boss was on top-left of the screen and the hardest was at the bottom-right so I started with Blast hornet and I was able to get through the stage,but I could not beat the boss,it was impossible and since I though this was the easiest stage,it left a really bad impression on me,I didn't touch the game for months until I've heard on the Internet that bosses had weaknesses and some stages are easier with certain power ups,so I went back and replay the game but I tried other stages and found out that you can upgrade X to make him stronger and that Blast hornet is one of the hardest to take on with the X buster alone. Knowing all that stuff made the game much more enjoyabe  :-)

When I first played KH1, I got trapped in the save game menu. I just kept pushing wrong button and wondering why nothing happens...

Also, I went till Hollow Bastion with Kingdom Key, because I didn't know I could change it.

last time i was at my friends we played sonic and all starts racing transformed im sonic hes knuckles i usually win but come last lap hes ahead by a little just then he gets one of the last power ups(a blow fish) and throws it behind him as he crosses the finish line causing me to run into it so heres were it gets bad just as im about to cross THE SECOND IM ABOUT TO tails...TAILS...speeds by me ;(

I was playing Mario Kart 8 with my brother. We were on Electrodrome. I was in first place, with my brother in second. End of the third lap. Everything was going well, until some random NPC far behind us tossed a green shell. It rebounded off the walls, and eventually got ahead of me. Time slowed down as I realized that its path and my path were about to cross... within three feet of the finish line, the green shell hit me, and my brother passed me. We were literally silent as we saw the shell about to hit me... And I went on shouting for a while about how stupid that was.

I died about 20 times trying to defeat the Pursuer, right at the beginning of the game when I was still under leveled because I was sure you needed to defeat him in order to continue, only to discover that he was essentially an optional boss.

 

I think my biggest one has to be not realizing that the crate in the central room floats up in the Water Temple in Ocarina of Time, and that you could go under it, because it added literal years to my attempt to beat the game.

When i played OOT3D for the first time, i got stuck in the first dungeon(in the Deku Tree i think) for hours because i didn't know you could break crates by rolling into them.

 

Also, i got lost in Atlantica the first time i played KH at a friends house.

I didn't even know he was an optional boss until after i reached the ruin sentinels and read the wiki. Apparently i was supposed to enter Bastille through No-man's wharf and the way through the pursuer is just the biggest waste of time in the game
